Traditional on- and off-exchange plans are not the only route. Many North County residents — especially the self-employed, independent contractors, and small-business owners — also qualify for true employer-level group coverage regulated under federal ERISA and Department of Labor rules. Qualifying as a bona fide employee takes only a few basic requirements — full-time work is not always required for some plan options — and pre-existing conditions are covered, just as with any employer group plan. These private PPO medical plans use nationwide networks like Cigna, Multiplan/PHCS, Aetna, First Choice Health, and in some states Blue Cross with national Blue Card, and enrollment is open year-round. Availability is subject to eligibility. Call us to see if you qualify.
If you get little or no premium tax credit, a private plan bought on or off the exchange — or an employer-level private PPO — is often the better value. If you qualify for meaningful savings, the marketplace usually wins. Since the enhanced federal subsidies expired at the end of 2025, many North County households are paying more than they need to. A quick comparison settles it.

North County is part of San Diego County’s pricing region, so the same six carriers apply for 2026: Blue Shield of California, Health Net, Anthem Blue Cross, Kaiser Permanente, Sharp Health Plan, and Molina. On the exchange only Blue Shield and Health Net sell PPOs; more PPOs are available off-exchange through brokers.
Many plans include one or more of them, but participation varies by plan and changes yearly. We verify your doctors and hospitals against each plan’s current network before you enroll.
Yes. We offer exclusive employer-group (ERISA) private PPO plans with nationwide networks like Cigna, Multiplan/PHCS, Aetna, and First Choice Health. Enrollment is open year-round, subject to eligibility.
For 2026, a 40-year-old typically pays about $516 to $690 per month for a Silver plan in San Diego County before subsidies. Your price depends on age, ZIP code, and household size.
